📋 Role Overview & Responsibilities
Our client is seeking Nurse Practitioners in Dallas/Tarrant Counties to perform health risk assessments through house calls. This ongoing contract job requires NP's to travel within a 90-minute drive within counties in any direction for 10/15 assessments a week. Scheduling is based on needs and NP’s availability, with a pay rate of $100 per assessment and mileage reimbursement.
The job entails conducting history and physicals at patient’s homes or in health facilities. Accurate and timely clinical documentation of acute, chronic, and long-term healthcare issues are required. The provider will educate patients and/or patient’s families, complete referrals to case management, nursing, pharmacy, etc., and participate in clinical meetings and compliance trainings.
The Nurse Practitioner must be Family Medicine board certified and have an active/unrestricted Texas NP license. Proficiency in MS and use of a personal car with adequate automobile insurance are required.
