Looking for Nurse Practitioners to complete Health Assessments in the greater Atlanta, GA area.

NPs are to travel to patient homes within the greater Atlanta, GA area. 10-15 visits/week. Scheduling based on needs and NP’s availability – Weekends are OK - Needs fluctuate.Availability: Assessments can be completed any day of the year (365 days) between 8:00 AM and 8:00 PM.Pay: $100 per assessment + Mileage reimbursementNo Shows (In-person): Pay $25

Job Description:Nurse Practitioners needed to perform health risk assessments which are essentially history and physicals done within the patient’s home or on occasion in a nursing home or other health facilityClinical documentation of acute, chronic, and long-term healthcare issues through patient encounters in EMR system; Documentation must be accurate, high quality, and completed promptlyProvider will ensure all appropriate consent forms are signed and datedProvider will have frequent interaction with patients, coders, schedulers, and supervising physicianThe provider will educate patients and/or patient’s family on chronic medical conditions, preventative care, and medication adherence complianceThe provider will complete referrals to case management, nursing, pharmacy, etc. as neededThe provider will participate in clinical meetings and compliance training which are primarily web-based; however, the provider will occasionally be required to attend on-site trainingProvider will not be required to prescribe medicine nor order any lab or diagnostic testing Provider will occasionally need to obtain samples for diagnostic screening tests such as finger stick for HgbA1C, LDL, and Lead levels; urine analysis may also be required based on client need.Provider will be responsible for reviewing patient records for past and present health issuesProvider will need to collaborate with the supervising physician and other members of the health care team Provider must be able to interpret laboratory data and diagnostic testing when available to assist in the diagnosis of medical conditionsAs part of the assessment, the provider must do ADL and Fall Risk evaluation, Depression and Nutritional Health Screening, BMI evaluation, and medication reconciliationPerforms other duties as assigned by the supervising physician such as performing examinations using equipment for diabetic retinal exams, bone density, etc.Assessments will be added to weekly timecards when documentation is completed (using the SSRS report)

Qualifications:Family NP required. Can consider Adult if no pediatric patients will be seen.Candidates must be board-certified by the ANCC or AANPMust have active/unrestricted state NP licenseNew graduates will be consideredMust be able to be credentialed by Medicare, Medicaid, and otherPrivate Insurance CompaniesRequired is proficiency with various computer-based applications such as MS Office (Logging into programs, MS Word, MS Excel, MS Outlook, general use of mouse and navigation of different programs, typing) along with utilizing various websites to perform the required duties.Strong background/interest in primary care and preventative medicine is a plusMust have use of own car with adequate automobile insuranceHighly motivated, self-directed professional with strong organizational skills and comfortable working independently dailyMust have a home internet connectionExcellent clinical assessment and analytical skills
