Speech Language Pathologist (PRN)

Job description

SUMMARY: To ensure quality and safe delivery of speech therapy services. The Speech Therapist will assist in the provision of home care services in accordance with Agency philosophy and established standards of home care and speech therapy practices.

Duties and Responsibilities:Participate in the development and maintenance of treatment plans.Assists the physician in evaluating home care patients to determine type of speech or language disorder and recommend appropriate corrective therapy.Participates with other clinical staff in assessing patient needs and developing an appropriate plan of care.Conducts initial evaluation independently if speech therapy is the only service ordered, as assigned.Completes speech therapy assessment note in a timely mannerParticipates in case conferences as assigned.Involves patients and family members in developing and implementing the plan of care whenever possible.Communicates recommendations and requirements for continuation and/or additional therapy services to appropriate individuals and obtain a physician order for any needed special equipment.Instructs other clinical staff members in methods of assisting home care patients to improve and correct speech disabilities.Implements current speech therapy practice.Provides rehabilitative services for speech and language disorders in accordance with physician orders.Helps home care patients adjust to physical limitations and maximize capabilities.Submits timely, complete clinical records documenting services rendered and observations about patient’s progress.Participates in discharge planning activities and prepares a written discharge summary.Supervises home health aides in lieu of a Registered Nurse if speech therapy is the only skilled service provided ad needed and as assigned.Implements appropriate infection control and safety measures and reports all hazards and accidents in a timely and accurate manner.Maintains confidentiality of patient information.Adheres to all Agency policies regarding schedule changes and/or missed visits.

Qualification and Experience:Minimum 1year recent experience as a Speech Therapist.Currently licensed to practice in the State of Virginia.Certified by the American Speech and Hearing Association

Physical Requirements:Ability to lift 35 - 45 pounds.Use hands to grip or feel.Reach with hands and arms.Occasionally required to bend, stoop and kneel.Frequently required to stand on one’s feet for a long period of time.Ability to deal effectively with stress.Must be able to work independently, make judgments based on assessments and data available and act accordingly.Must be flexible, innovative and possess good interpersonal skills. Must be able to cope with mental and emotional stress and demonstrate emotional stability.Visual/hearing ability sufficient to comprehend written/oral communicationWorking Conditions:Works in office areas as well as in client homes and in assisted and independent living facilities. Interacts with clients, family members, staff, visitors, government agencies/personnel, etc., under all conditions/circumstances.Is subject to hostile and emotionally upset clients, family members, staff, visitors, etc. under all conditions/circumstances.May be exposed to infectious waste, diseases, conditions, etc.Risk level- LowJob Types: Part-time, Temporary
