Bilingual Medical Case Manager
Pay Rate: $21/hr.
Location: Houston, Texas 77006
Hours: Monday – Friday 8:00am – 5:00pm

Responsibilities
Assist Medical Case Management coordinator by assessing potential clients for eligibility and providing intake and individualized service plans. Provide quality service to clients and interpreters when necessary.Address identified needs through medical case management services and advocate for clients with clients' doctors and medical health professionals.Perform other duties as assigned such as providing health literacy workshops, training, and outreach to the Community.Attend meetings, staff retreats, tours, fairs, speaking engagements, and events.
List Qualification Or Requirements
Must be bilingual in English and Pashto or Dari.High School Diploma or GED.One (1) year experience in an administrative setting or within refugee populations.Valid Texas driver’s license.Advanced in Microsoft Word Office Suite (Word, Excel or Access).
