Case Manager (Mental Health Facility)

United Recovery Project is seeking a professional, dedicated, and compassionate Case Manager to join our outstanding Clinical Team. Candidate must have a thorough understanding of mental health disorders, and have the skills and compassion necessary to work in this environment. Core Responsibilities of the Case Manager:Coordination of case management services.Completes assessments, insurance and FMLA documents, and provides discharge and aftercare services.Communicates with law enforcement and probation, employers and government agencies on behalf of the patient.Builds relationships with community partners to provide clients with referral and linkage to community resources.Acts as a liaison between the multidisciplinary team and other departments, families and other outside agencies to advocate for the patient and remove barriers to care.Assists with ACA (Against Clinical Advice) blocking and patient management.Participates in case staffing's and reports concerns.Reporting Suicidality/Homicide of a patient based to their job position.

Pay Range $40,000 to $62,000 annually commensurate with experience.

Genesis House Requirements

Qualifications of the Case Manager:Valid and active Driver License is required.Computer literacy required.A minimum of 2-year experience in Mental Health required.A minimum of 1-year experience in Case Management prefered.Highschool Diploma or GED required.KIPU EMR knowledge preferred.Current CPR certification and First Aid preferred.Ability to work overtime, weekends, holidays, and flexible hours.Applicants are required to successfully clear a Level 2 Background check.
