Care Manager:Veta Health is seeking a health tech-enthusiastic RN Care Manager to virtually support patients. Veta Health provides care services on behalf of its customers’ clinical teams. As part of our Clinical Services Department, an RN Care Manager will ensure an exceptional care experience by leveraging our digital platform to monitor patient data, escalate issues, document, and provide patient education.

Reporting to a Veta Health Clinical Services Manager, the ideal candidate is personable, compassionate, and excited about the intersection of healthcare and technology. Successful candidates will share in our vision, are comfortable working in a fast-paced environment, and are self-starters.

This position is a remote position with preference for East coast/Midwest- based candidates.

Responsibilities:Deliver outstanding virtual care experiences to patients and their care teamsContribute strategically to optimizing AI-based clinical workflows, driving efficiency in collaboration between clinical staff and patientsEnroll, consent, onboard, and manage patients in various virtual care programs (e.g., remote physiologic monitoring, chronic care management, etc.)Monitor patients for symptoms, treatment side effects or complications. Support medication treatment plans and adherence. Escalate to the patient’s care team as warranted.Execute and coordinate patient’s treatment plan with patient’s physician/APRN/PA.Conduct assessments to develop a care management plan. Empower patients in self-management of their conditionsDrive ongoing engagement with Veta Health's softwareProvide supervision and support to LPN nursing staffGuarantee the delivery of top-tier patient care and actively participate in cultivating an environment conducive to enhancing processes and quality improvementDemonstrate excellent communication, organization, and leadership skillsComplete thorough and timely documentationPerform other duties as assigned by the Clinical Services Manager

Qualification/Experience:Education: Minimum of an Associate Degree in nursing is required and a Bachelor's Degree in nursing is preferred. Nursing degree from a nursing-accredited college or university is required. Licensure: Current, active Registered Nurse Michigan license is required. Must be eligible for unrestricted Registered Nurse licensure in all states.Qualification/Experience (required): 3+ years of recent Registered Nurse experience in acute care or ambulatory care. 2+ years supervisory experience preferred. Tech native. Demonstrated proficiency in Google Workspace, including Sheets, Drive, Gmail, Meet, and DocsQualification/Experience (preferred): Virtual Care/ Telehealth/ RPM, Care management, population health, and/or chronic disease management experience is preferred
