Community Specialist

Company Description Visiting Nurse Health System is a nonprofit organization dedicated to providing quality, patient-centered care to seniors in the Atlanta Metropolitan Area. For over 75 years, our nurses, therapists, and clinical staff have been helping patients manage their health and recover from illness in the safety and comfort of their own homes. We also offer Hospice Atlanta services to terminally ill patients, providing compassionate care in their own homes. As the largest nonprofit home healthcare and hospice provider in Georgia, we serve over 6,000 patients and their families daily in 31 counties.

Role Description This is a full-time role as a Hospice Community Specialist. As a Hospice Community Specialist, you will be responsible for performing a variety of tasks to support the Hospice Atlanta program. This includes engaging with the community, providing information and support to patients and families, coordinating care services, and ensuring that patients receive compassionate care in their homes. We are looking to fill a Cobb/Cherokee counties position.

Qualifications Excellent interpersonal and communication skillsExperience in community engagement and customer serviceAbility to effectively manage and coordinate care servicesStrong problem-solving and decision-making abilitiesKnowledge of hospice care and end-of-life support servicesExperience in community management and outreachCompassionate and empathetic approach to patient careBachelor's degree in a related field (Social Work, Healthcare, etc.) is preferredCommunity relationships that are active.Hospice or home health sales experience preferred.

Relevant skills and qualifications that would be beneficial include knowledge of healthcare regulations and guidelines, experience working with diverse populations, and proficiency in relevant software and technology.
