Care Coordinator LPN - Geriatrics - NEMG

Performs under the direction/supervision of a physician/doctor, advanced practice registered nurse, and/or registered nurse, in administering treatments prescribed by a legally authorized physician/doctor, or advanced practice registered nurse. Performs duties within the framework of supportive and restorative care, health counseling and teaching, case finding and referral, in the implementation of the total health regimen. Executes the medical regimen under the direction of a licensed physician/doctor, and/or advanced practice registered nurse while implementing a medical plan of care. Customer Service Qualtiy: Effectively works in collaboration with physicians/doctors, advanced practice registered nurses, registered nurses, family members, caregivers, and visiting nurse services to implement predetermined plan of care/guidelines for each assigned pt/client 1.1 Communicates this plan to all members of the patient care team, including physicians/doctors, advanced practice registered nurses, family members, caregivers, and visiting nurse services.Customer Service Qualtiy: Provides direct/indirect care to patients and families. 2.1 Interacts effectively with patients and families as evidenced by observation and feedback from staff and customers.Financial/Volume Results: Maintains patient records effectively. 3.1 Process electronic and/or hard copy patient records to reflect changes in patient condition, medication changes, new diagnoses, physician/doctor, and/or nurse practitioner encounters as evidenced by documentation.Financial/Volume Results: Participates in data collection aspects of quality, utilization, and satisfaction data review. 4.1 Updates and maintains census records and provides those records to designated recipient when requested.Department Goal: Utilizes relevant research findings to support and advance nursing practice and improve patient outcomes. 5.1 Attends 8 hours continuing education per review year. At least 4 hours must be related to geriatrics.Department Goal: Achieve established performance and professional goals and objectives. 6.1 Achieve personal performance and professional goals developed annually.May assist and/or perform chaperone duties for patient medical procedures as needed. Qualifications EDUCATION Graduation from an accredited Practical Nursing program EXPERIENCE 3-5 years clinical experience in the area of specialty. Strong interpersonal and organizational skills. BLS preferred LICENSURE Current LPN licensure in the State of Connecticut PHYSICAL DEMAND MMR is required. Varivax (chicken pox vaccine) or evidence of prior chicken pox is required. Hepatitis B (or signed declination) is required for those with potential exposure to blood/body fluids. Tdap and influenza vaccination are strongly encouraged. YNHHS Requisition ID 117406
