Description
Zumbro Valley Health Center is seeking a Full-time Registered Nurse (RN) to support the outpatient Psychiatry/Integrated Clinic and Residential Treatment programs.
The RN is responsible for providing a variety of clinical and therapeutic services as well as coordinating integrated medical and behavioral healthcare. The RN works as a member of a multidisciplinary team to provide the highest quality clinical care. Primary responsibilities include health screening and assessment, triage, provision of staff and client health education, comprehensive care management, and facilitation of referral, coordination and follow up for health and social services. The RN provides direct support for physicians, nurse practitioners, and psychiatrists, and provides medication/health education services to staff and clients.
Key Responsibilities
Complete and document initial and ongoing health screening and assessments in the outpatient and residential treatment settings Monitor health and medication related policies and procedures in the Residential Program, including response to medical emergencies, medication administration, and review of medication errors and training of staff Complete triage with appropriate documentation, referral, and follow up Provide staff and client education regarding medical and behavioral health conditions, medications, and healthy lifestyle habits in both individual and group settings Assist with transitional care/discharge planning for clients admitted to the emergency department, hospital or other treatment settings; coordinate with the client and internal and external treatment teams on recommendations and ensure timely follow-up services Provide crisis intervention and make referrals to necessary crisis services Provide nursing cross coverage in other agency programs as indicated
Qualifications
Requirements and Qualifications
Computer and technical skills required including use of Microsoft Office (Outlook, SharePoint, Teams, Excel), telehealth video platforms, typing, patient portals, and navigating an electronic medical record Strong organizational and time management skills Ability to work independently and effectively manage a variety of tasks and responsibility with minimal supervision or direction Strong attention to detail and follow-through
Education and Experience
Licensed by the State of Minnesota as a Registered Nurse Three years of mental health nursing experience preferred Experience working with a multidisciplinary team Experience using an electronic medical record
Hours: Monday-Thursday 8-5:30pm or 7:30-5pm, Friday 8-12pm. No evenings, weekends, or holidays.
